Application Protocol for OASIS® Ultra
Tri-Layer Matrix
Prepare 1. Prepare
Prepare the wound bed to make it free of exudate and devitalized tissue. Wait for any bleeding to stop before applying OASIS® Ultra.
Apply 2. Apply
Measure the wound and select the appropriately sized sheet of dry OASIS® Ultra. Cut the sheet to size and position it to cover the wound surface, extending slightly beyond the margins. If multiple sheets are necessary, overlap the edges slightly.
Anchor 3. Anchor
As required, anchor OASIS® Ultra using the physician’s preferred fixation method (eg, Steri-Strip®, tissue sealant, bolsters, or dissolvable clips).
Rehydrate 4. Rehydrate
Gently rehydrate OASIS® Ultra with sterile saline.
Dress 5. Dress
Apply a non adherent primary wound dressing over OASIS® Ultra (eg, Adaptic®, Vaseline® Petroleum Gauze, or Mepitel®). The primary dressing is usually changed on Day 7 to avoid product disruption.
Cover 6. Cover
Select an appropriate secondary dressing, based on the type of wound and the amount of exudate. Use a multi-layer compression bandage if necessary.
Reassess 7. Reassess
Assess the wound after 7 days or as necessary. The wound should be free of infection and necrosis. Carefully remove any remaining loose product from around the wound edges as needed.
Reapply 8. Reapply
Reapplication is typically necessary every 7 days. Reapply as needed if OASIS® Ultra is no longer covering the wound and it is free of infection and necrosis, repeating steps 2 through 8 as appropriate. If a gel forms on the wound surface, do not attempt to forcibly remove. Gently rinse the wound surface, leaving the extracellular matrix (ECM) gel intact. See below for details on caramelization.

What to Expect: Caramelization

  • Over time, OASIS® Ultra may form a caramel-colored or off-white gel on the wound
  • Do not forcibly remove this gel by debriding
  • This caramelization contains extracellular matrix (ECM), which continues to replace deficient and missing ECM in the wound
  • Gently cleanse the wound surface with sterile saline, then reapply OASIS® Ultra if necessary


  • If infection; chronic inflammation; allergic reaction; or excessive redness, pain, swelling, or blistering occur, OASIS® Ultra should be discontinued
  • Contraindications: This device is derived from a porcine source and should not be used in patients with known sensitivity to porcine material. OASIS® Ultra is not indicated for use in third-degree burns
